Come in for a sick visit, physical, or chronic care follow-up. Telehealth is available when appropriate.

3. NEXT STEPS & FOLLOW-UP

Get a clear plan, any needed labs or screening, and simple follow-up through the patient portal.

We accept many major insurance plans. Coverage varies by plan—please call our office and we’ll help confirm your benefits.

Photo ID, insurance card, a list of medications, and any recent labs or records you’d like us to review.

For routine refills, please use the patient portal or call our office. Refill requests may take 24–48 business hours. Controlled medications may require an appointment.

Same-day visits may be available depending on schedule. Please call early for best availability

Telehealth is available for certain visit types when appropriate. Call us and we’ll guide you to the right option.
